يعد سرطان المعدة خامس أكثر السرطانات انشاراً والثالث في التسبب بالوفيات على مستوى العالم. وبالرغم من توفر توجهات علاجية متعددة لعلاج هذا النوع من السرطان الا أن التوقعات العلاجية للمرض لازالت سيئة اذا ما قورنت بأنواع أخرى من السرطانات.
في الأونة الأخيرة تم تسليط الضوء على العلاج المناعي للسرطان باستخدام جزيئات مثبطة للمناعة أهمها
"مستقبل الموت الخلوي" PD-1 و لجينه: لجين الموت الخلوي المبرمج (PD-L1). بناءاً على ذلك, تم استهداف الحاجز المناعي (PD-1/PD-L1) كنوع علاجي جديد كما أنه من المتوقع أن يكون انتشار تعبيره في المحيط السرطاني علامة تنبؤ بيولوجية لمدى الاستجابة لهذا النوع من العلاج.
يھدف ھذا البحث إلى تحديد نسبة ظھور هذا البروتينPD-L1 على خلايا المحيط السرطاني في سرطان المعدة, عند مجموعة من المرضى الأردنيين و بحث اقترانه بعوامل سريرية مرضية. شمل البحث 96 حالة شخصت بسرطان المعدة. و استخدمت تقنية الكيمياء النسيجية المناعية(Immunohistochemistry ) لتحديد مدى انتشار اللجين بكل حالة بحسب ارتباط أجسام مضادة للجين PDL-1باستخدام(VENTANA PD-L1 (SP263) Rabbit Monoclonal Primary Antibody).
وتم اعتماد نظام التقييم الايجابي المشترك (combined positive score (CPS)) ≥1 لتقييم مدى ايجابية الخلايا المعبره للجين.
وجدت الدراسة أن بروتين (PD-L1) ينشتر تعبيره في 66.7% من حالات سرطان المعدة, كما تم استنتاج عدم وجود اي اقتران بين انتشار تعبير اللجين بعوامل سريرية مرضية, الا أنه وجد أن هذا البروتين يعتبر عامل تنبؤ مستقل ويشير الى وجود تحسن في منحنى البقاء على قيد الحياة.
Gastric cancer (GC) is the 5th most common cancer, the 3rd leading cause of cancer related mortality worldwide. Although multidisciplinary therapeutic strategies have improved treatment outcome, the overall prognosis for gastric cancer patients remains poor. Recently, immunotherapeutic agents targeting immunosuppressive proteins (immune checkpoint inhibitors that allow cancer cells to evade the immune system, such as programmed death-1 receptor (PD-1) and its ligand, PD-1 ligand 1 (PD-L1) ) have been investigated as potential treatments for cancer, among which are the anti PD-1 and anti PD-L1. Targeting the PD-1/PD-L1 immune checkpoint signaling has been identified as a novel promising treatment strategy in several tumor entities, and it is suggested to be a predictive biomarker for PD-1/PD-L1 checkpoint inhibitor treatment response. This study evaluated the expression of PD-L1 in gastric cancer samples from the Jordanian population, and assessed its correlation with certain clinicopathological factors. A total of 96 cases diagnosed with gastric cancer were included in the study. Immunohistochemistry assay was employed for the evaluation procedure using (VENTANA PD-L1 (SP263) Rabbit Monoclonal Primary Antibody) and the scoring was based on a combined positive score (CPS) ≥ 1. It was found that 66.7% of the study samples were positive for PD-L1. The expression of PD-L1 was not significantly associated with any of the assessed clinicopathologoical factors, however, it was considered an independent favorable prognostic factor for overall survival (versus PD-L1 negative; hazard ratio, 0.481; 95% confidence interval, 0.231- 1.001; p= 0.050).
